Sildenafil is an oral drug for the treatment of male erectile dysfunction.

Mechanism of action:

Erection of penis physiologically involves release of nitric oxide (NO) in the corpus cavernosum during sexual excitement.No then activates the enzyme gaunylate cyclase, which results in increased level of CGMP. This produces relaxation of smooth muscles in corpus cavernosum and allowing inflow of blood.

sildenafil is rapidly absorbed.The peak serum concentration is reached within 30-120 minutes.

Administration of sildenafil with high fat meal delays the rate of absorption and cma.

sildenafil is metabolized in the liver and is excreted predo-minantly in the feaces.The terminal half life of sildenafil is about 4 hours.

Healthcare providers contraindicate MTM in patients with a known allergy to any component of the tablet

There is potential for cardiac risk of sexual activity in patients with a pre-existing cardiovascular disease. Healthcare providers do not recommend MTM for men whose underlying cardiovascular conditions make sexual activity inadvisable

Precautions:

Healthcare providers must prescribe MTM with great caution in the following groups of patients. The treating physician must decide its use taking into consideration risk benefit ratio in  following group of patients:

• Patients with history of myocardial infarction, stroke of life threatening arrhythmia
• Patients with hypertension
• Patients with cardiac failure, coronary artery disease, unstable angina
• Patients with genetic disorders of retinal phosphodiestrases or patients with retinitis pigmentosa.

Adverse reaction:

Use of sildenafil may cause:

Headache, flushing, dyspepsia, nasal congestion, urinary tract infection, abnormal vision, diarrhoea, dizziness and rash.

Dosage

usual recommended dose is 50mg, taken one hour before sexual activity.

Based on clinical results and patient tolerance, healthcare providers may increase the dose to 100 mg or reduce it to 25 mg.

The maximum recommended dosing is once per day
