Plavix 75 mg is a brand-name medication that contains the active ingredient clopidogrel, an antiplatelet agent. It is primarily used to prevent blood clots in people at high risk of cardiovascular events.

Key Information about Plavix (Clopidogrel):

Mechanism of Action:

  • Clopidogrel inhibits platelet aggregation by irreversibly blocking the P2Y12 ADP receptor on platelets.
  • This prevents platelets from sticking together, reducing the risk of forming harmful clots in arteries.

Indications:

Prevention of Atherothrombotic Events:

    • In patients with a history of myocardial infarction (heart attack), ischemic stroke, or established peripheral arterial disease.

Acute Coronary Syndrome (ACS):

  • Used with aspirin for patients with unstable angina or non-ST-elevation myocardial infarction (NSTEMI).
  • In ST-elevation myocardial infarction (STEMI), it is used as part of a dual antiplatelet therapy (DAPT).

Dosage:

  • The typical dose is 75 mg once daily, with or without food.
  • For ACS, an initial loading dose (e.g., 300 mg) may be required, followed by a daily maintenance dose.

Side Effects:

Common side effects include:

  • Bleeding (e.g., nosebleeds, bruising, or more serious internal bleeding).
  • Diarrhea.
  • Rash.
  • Gastrointestinal discomfort.

Rare but serious side effects:

  • Thrombotic Thrombocytopenic Purpura (TTP): A rare blood disorder causing small blood clots, low platelets, and anemia.
  • Severe bleeding.

Warnings and Precautions:

Bleeding Risk:

  • Avoid use in patients with active bleeding (e.g., peptic ulcers, intracranial hemorrhage).
  • Monitor for signs of bleeding, especially when used with other anticoagulants or antiplatelet drugs.

Reduced Effectiveness in Poor Metabolizers:

  • Patients with certain genetic variants (e.g., CYP2C19 polymorphisms) may not convert clopidogrel into its active form, reducing its efficacy.

Surgery:

  • Discontinue 5-7 days prior to elective surgery to minimize bleeding risk.

Drug Interactions:

  • Proton Pump Inhibitors (PPIs): Some (e.g., omeprazole) can reduce clopidogrel’s effectiveness by inhibiting CYP2C19.
  • Other antithrombotics or NSAIDs increase bleeding risk.

Monitoring:

  • Regular checks for signs of bleeding.
  • Consider genetic testing for CYP2C19 function in some patients.
